How is
Transitional Care Different from Acute Care?
When you first came to the hospital you
were acutely ill and care was directed at treating your illness.
As your condition improves, you need to continue to get well.
The Transitional Care Program focuses more on getting well after
your illness has been treated.
In our program you are encouraged to wear your own clothes, eat
your meals in the activities room, and take part in a variety of
daily activities. You receive therapy to meet your individual
needs. Your family doctor, as part of your health care team,
will continue to monitor your progress.
Who Qualifies for
the Program?
You must have at
least a 3 day stay as an acute care patient in a hospital prior
to entering the Transitional Care Program. You must also need
ongoing monitoring and/or require rehabilitative therapy.
Bringing Families
Together
Too often families
are separated because patients must go to a larger city, perhaps
many miles from their homes. Our program allows a patient to
remain in his or her community while still receiving the best
possible care.
The Inpatient
Transitional Care Bed Program Treats:

Patients admitted
from another hospital to complete their recovery

Patients who have
been hospitalized and need additional time to recover before
they are discharged

Patients
recovering from major surgery or joint replacement
-
Patients
recovering from a major accident or stroke
-
Patients
suffering from wounds which are not healing or need special
care

Daily therapy is
centered on self care skills and body strengthening, which
will increase your independence

room as well as planned daily activities are available to
the resident
